Job Description: The Care Coordinator manages inquiries for proton therapy from a variety of sources including referring providers, prospective patients, caretakers, and others. The Care Coordinator obtains pertinent information and medical records to assist in the appropriate scheduling of patient consultations. The Care Coordinator works closely with the Radiotherapy Team to coordinate and schedule patients for internal and external services, optimizing the progression of care by closely monitoring and following up on diagnostic and therapeutic procedures. The Care Coordinator enhances the experience of prospective and established patients and their caregivers through timely response and development of a compassionate and professional rapport. ESSENTIAL FUNCTIONS:
- Promptly answers and responds to phone, fax, and web inquiries.
- Greets callers in a courteous, efficient manner. Routes calls and messages to appropriate employee or department.
- Provides general information on proton therapy to inquiring individuals.
- Promptly obtains medical records and insurance information for all prospective patients.
- Adheres to established guidelines for case review with the Radiation Oncologist to determine candidacy for treatment.
- Promptly schedules appropriate patients for consultation.
- Manages and optimizes utilization of consultation appointment availability.
- Attends Care Coordination team meetings.
- Provides appropriate information packets and guidance on cancer support services to prospective patients.
- Follows up with prospective patients within established timeframes.
- Accurately registers prospective and new patient information to the appropriate clinical application.
- Establishes and maintains excellent patient-focused working relationships with internal and external care team members.
- Processes incoming patient health records.
- Promptly coordinates and schedules patient diagnostic and treatment procedures as ordered by the provider.
- Coordinate and schedules referrals to medical specialists and supportive care services.
- Processes incoming and outgoing medical records as necessary for coordination of care.
- Promotes patient advancement through the plan of care, relaying information between patients and clinical providers to support timely decision and progress.
- Promptly retrieves and responds to phone, fax, and portal communication.
- Demonstrates excellent customer service, responding promptly to patient and team member requests.
- Promotes positive patient experience, encouraging patients and involved caregivers to participate in patient satisfaction surveys.
- Exercises problem-solving and conflict resolution skills when handling patient complaints; refers patient complaints to appropriate designated personnel as needed.
- Functions as a preceptor to newly hired staff.
- Adheres to all Tennessee Oncology policies and procedures.
- Performs other duties as assigned. REQUIRED QUALIFICATIONS:
- High school diploma or GED
- Minimum of five years of professional office experience or equivalent experience in a customer service–oriented industry
- Proficiency with computer technology and common office software
- Strong professional presence with excellent verbal and written communication skills
- Demonstrated reliability and dependability
- Effective task prioritization PREFERRED QUALIFICATIONS:
- Tertiary degree or diploma in a related area of study
- Conflict management
- Knowledge of medical technology or EMR use
