We're looking for a Medical Social Worker (MSW) to provide psychosocial support to home health and hospice patients and families throughout Salt Lake County. This is a part-time position with a flexible schedule , ideal for a social worker who values meaningful patient interaction, professional autonomy, and the ability to organize visits around patient needs. Qualifications Required:

• Master's degree in Social Work (MSW) from an accredited social work program
• Current Utah social work license in good standing (CSW or LCSW)
• At least 1 year of social work experience in a healthcare setting
• Strong assessment, communication, and interpersonal skills
• Ability to work independently in patients' homes
• Valid driver's license and reliable transportation Preferred:
• Home health or hospice experience
• Palliative or end-of-life care experience
• Experience with grief, bereavement, serious illness, or community resources About the Role As a Medical Social Worker, you'll help patients and families navigate the emotional, social, financial, and practical challenges that can accompany illness, disability, and end-of-life care. You'll work directly with patients and families in their homes while collaborating with nurses, therapists, physicians, chaplains, and other members of the interdisciplinary care team. Responsibilities
• Complete psychosocial assessments for home health and hospice patients
• Identify emotional, social, financial, caregiver, and community-resource needs
• Provide counseling, education, and supportive interventions within your scope of practice
• Connect patients and families with community resources and available benefits
• Provide crisis intervention and support when appropriate
• Assist with advance-care planning and advance directives
• Assess caregiver stress, coping, grief, and bereavement needs
• Advocate for patients and families
• Participate in hospice interdisciplinary team meetings and care planning
• Assist with discharge planning and transitions of care
• Identify psychosocial, cultural, and spiritual concerns and coordinate additional support when appropriate
