Cabrillo Hospice is seeking a compassionate and dedicated Social Worker to join our interdisciplinary hospice team and make a meaningful difference in the lives of patients and caregivers. At Cabrillo Hospice, we are committed to providing patient-centered, dignified end-of-life care with a strong focus on emotional support, family education, and holistic well-being. What You’ll Do Conduct Comprehensive Psychosocial Assessments Assess the emotional, social, and environmental needs of patients and families, including caregiver stress, family dynamics, coping abilities, and safety concerns. Provide Emotional Support & Counseling Offer compassionate counseling to patients and families navigating serious illness, grief, and end-of-life transitions. Collaborate with the Interdisciplinary Team Work closely with nurses, physicians, chaplains, and other care team members to develop and support individualized plans of care. Connect Families to Community Resources Serve as a liaison to community agencies and support services to ensure patients and families have access to appropriate resources. Bereavement Support Participate in and support hospice bereavement services, providing follow-up and resources to families after the loss of a loved one. Ensure Compliance & Quality Care Maintain accurate documentation and ensure services align with hospice regulations, policies, and standards of care. What We’re Looking For Master’s Degree in Social Work (MSW) Hospice or healthcare experience preferred (geriatrics or end-of-life care a plus) Strong interpersonal and communication skills Compassionate, patient-centered approach Ability to work independently while collaborating within a multidisciplinary team Cultural sensitivity and respect for diverse backgrounds and belief systems Why Cabrillo Hospice Meaningful Work: Make a direct impact on patients and families during critical moments.
