• 4:30 PM Potential for limited weekend and holiday coverage Help Patients Access the Rehabilitation Care They Need Vanderbilt Clarksville Hospital is seeking an Admissions Coordinator to support our inpatient rehabilitation services. This role serves as a key liaison between referring providers, patients, families, case managers, and the rehabilitation team to facilitate timely and appropriate admissions into rehabilitation services. The Admissions Coordinator evaluates referrals, gathers clinical and insurance information, coordinates the admission process, and helps ensure patients are transitioned efficiently into the appropriate level of rehabilitative care. This position plays an important role in optimizing patient access, supporting care coordination, and contributing to positive patient outcomes. Key Responsibilities
• Review and evaluate patient referrals for rehabilitation services.
• Coordinate admissions and patient transitions in collaboration with physicians, care managers, therapists, nursing staff, and other healthcare professionals.
• Verify patient demographics, insurance eligibility, benefits, authorizations, and required documentation.
• Communicate with patients and families regarding the admission process, expectations, available services, and next steps.
• Facilitate timely collection of medical records, clinical information, and supporting documentation.
• Initiate and maintain accurate documentation within the electronic medical record (EMR).
• Assist with care coordination activities to promote efficient patient throughput and successful admissions.
• Monitor admission workflows and help identify opportunities for process improvement.
• Ensure compliance with regulatory standards, privacy requirements, safety practices, and organizational policies.

Qualifications Required
• Graduate of an approved discipline specific program.
• One year of experience in relevant rehabilitation healthcare setting
• Strong communication, organizational, and customer service skills.
• Basic Life Support (BLS) certification through the American Heart Association within 90 days of hire. Preferred
• Experience with insurance verification, authorization processes, and medical documentation. Required Licensure

• Collaborates with physicians, care managers, and other healthcare staff to ensure accurate and timely patient admissions, including necessary documentation and care planning.
• Verifies patient information, including demographics, insurance, and medical records, to ensure compliance with facility admission protocols.
• Communicates effectively with patients and their families, providing information about the admission process, care expectations, and patient rights.
• Supports the coordination of care by initiating orders, treatments, and diagnostic tests as appropriate, based on the admission protocol.
• Maintains accurate and complete documentation of all admission activities in the electronic medical record (EMR), adhering to facility standards and regulations.
• Participates in quality improvement initiatives related to the admission process, identifying opportunities for enhancement and contributing to implementation efforts.
• Provides patient education on facility services, treatment plans, and post-admission care to ensure patient understanding and satisfaction.
• Ensures compliance with all regulatory requirements, including privacy, safety, and infection control, during the admission process.
