• Evaluate patients’ physical, cognitive, and functional abilities to identify areas that may affect their independence and quality of life
• Develop individualized treatment plans based on each patient’s needs, goals, and abilities
• Provide therapeutic interventions to improve patients’ ability to perform activities of daily living
• Help patients develop and maintain skills needed for bathing, dressing, grooming, meal preparation, and other daily activities
• Recommend and teach the use of adaptive equipment and techniques to promote independence and safety
• Assess patients’ homes and recommend modifications to reduce fall risks and improve safety
• Provide exercises and therapeutic activities to improve strength, coordination, balance, mobility, and functional independence
• Educate patients and caregivers on treatment plans, safety techniques, and strategies for continued progress at home
• Monitor patient progress and modify treatment plans as needed
• Collaborate with physicians, nurses, physical therapists, speech therapists, and other healthcare professionals
• Maintain accurate and timely documentation of patient evaluations, treatment plans, progress, and services provided
• Promote patients’ independence, dignity, and quality of life through compassionate, patient-centered care
• Follow all applicable professional, organizational, and regulatory standards Requirements
• Proven work experience as a Physical Therapist
• Intensive education and clinical expertise
• Ability to manage patients with different types of personalities
• Current knowledge of treatment practices
• Drive to continually learn and grow

• Excellent interpersonal communication skills
• Valid license to practice physical therapy
