Description Summary: Case Manager is responsible for implementing day-to-day and long-term case management services to residents of Guadalupe Home, both in the home and out in the community when applicable. Assess needs of each resident and her child, assisting in the development of comprehensive service plans for the assigned residents. This role also provides case management services to clients in an integrated, multi-cultural setting. The Case Manager will work with community resources and other community-based providers/representatives to coordinate, monitor, evaluate, and advocate for services to meet each resident’s specific needs in tracking performance measures and goal path created for each resident. Requirements Position Responsibilities: • *Pass TPCN Counselor Certification exam with 80% or better annually/as required • *Properly bill TPCN and/or other agency utilized systems provided to residents • *Gain a working knowledge of staff policies and guidelines. • *Maintain positive culturally competent relationships with residents and staff. • *Utilize training in trauma informed consequences when redirecting resident behavior.

  • Documents and reports to Program Director on resident progress, maintain statistical records, and perform other record keeping functions as required by the agency in files and through data entry, including agency software and TPCN.
  • Exercises time management skills to provide efficient and productive case management services to all residents.
  • May attend court hearings, TDFPS staffing’s, Circle of supports, etc. for residents on their assigned caseload.
  • Ensure all resident files/records are current with appropriate documentation included, such as admission assessments, service plans/reviews, case notes, goals/reviews, current court orders and appropriate consents and agreements; work/school documentation, educational testing, discipline referrals, etc.
  • Responsible for inputting information; admission and discharge resident data, and measurement data via agency data bases.
  • Assist in development and facilitate implementation of admission assessments and service plans to ensure quality care and compliance.
  • Assist in planning and coordinating intervention strategies appropriate for each resident’s identified needs. Develop new goals and strategies as needed.
  • Adopt a proactive stance on behalf of each resident on his/her caseload. Ensure all residents served have appropriate advocacy support, internally and externally.
  • Refer to community agencies as necessary and appropriate, including referrals to Medicaid, WIC, etc.
  • Actively participate in all team meetings, to ensure quality care and accurate and informed services by all team members. Assist in special projects, activities, and events as needed.
  • Attend local seminars, conferences, and training to increase knowledge and skill for professional development.
  • Assist with driving residents to appointments when needed.
  • Meet with clients on a minimum of a weekly basis and adjust their schedule to ensure all clients receive case management convenient to the various client’s schedules
  • Continually assess ongoing changes in behavior, circumstances, or conditions that may affect child safety.
  • Complete measurement and outcomes and client goals in agency data base.
  • Demonstrate client progress toward risk reduction, achievement of treatment goals, and positive case outcomes
  • Assist with implementation of social and recreational activities for residents as needed.
  • Must complete all required professional development assigned in a timely manner
  • Must be sensitive to the service population’s cultural and socioeconomic characteristics.
  • Adhering to safety training and protocols on a daily basis, and taking precautionary measure to ensure the safety and well-being of self, and others.
  • Responsible for protecting the confidentiality of any information or material obtained in the service with the organization to include but not limited to client names and information, services rendered to clients, donors names and gifts, internal and external investigations or results of any investigations, and financial information.
  • Adherence to the Code of Conduct and the Faith and Moral Policy is mandatory.

  • Other duties as assigned by the Program Director, Senior Director or Vice President. Competencies: Advocacy Adaptability Communication Interpersonal Awareness Thorough Minimum Qualifications:
  • Education
  • Bachelor’s degree in social work, behavioral sciences or related field.
  • Experience
  • Minimum of 1-year volunteer or paid experience working as a case manager with vulnerable populations in a child welfare setting or Non-profit setting.
  • Minimum of 3 years’ case management experience may be substituted for Bachelor’s degree.
  • License and Credentials
  • Must be at least 21 years old
  • Have a valid driver license and clean driving record
  • Have valid vehicle liability insurance
  • Ability to pass DFPS, DPS, and Sex Offender background checks
  • Bilingual English/Spanish preferred Minimum Knowledge and Skills:
  • Extensive working knowledge of trauma informed care and client advocacy
  • Experience with computer software using Microsoft Suite and Adobe
  • A solid grasp of child development and adolescents
  • Must be detail oriented, organized, self-motivated, work well independently and on a team;
  • Must have good written and verbal skills;
  • Must have good critical thinking and problem-solving skills. Travel Requirements: Travel requirements for the position includes 25% local and 0% overnight.
