Registered Nurse - Pre- Liver Transplant Coordinator

• Pre- Liver Transplant Coordinator The Pre- Liver Transplant Coordinator manages and coordinates the complete evaluation, education, and waitlist preparation phase for patients with end-stage liver disease who need a liver transplant. What You Will Do Patient Evaluation & Workup
• Coordinates comprehensive pre-transplant clinical evaluations for prospective patients. Reviews diagnostic tests, blood work, imaging, and specialty consults to determine transplant eligibility Multidisciplinary Collaboration
• Serves as the primary liaison between patients, families, hepatologists, transplant surgeons, social workers, dietitians, and financial counselors. Prepares and presents clinical candidacy data at medical review board selection meetings. Waitlist Management & Regulation
• Ensures compliance with national and federal standards, including guidelines set by UNOS (United Network for Organ Sharing) , CMS (Centers for Medicare & Medicaid Services), and the Joint Commission. Maintains and updates patient status on the national transplant waiting list. Patient Education & Triage
• Educates patients and their families on the entire transplant journey, surgical risks, and pre-/post-transplant expectations. Triages incoming clinical calls, manages symptoms, and assists with complex medication management. Administrative & Quality Assurance
• Prepares quality improvement (QI) reports, tracks clinical outcome metrics, participates in regulatory audits and surveys, and supports transplant program administration. What You Will Need
• Bachelor's degree in nursing (BSN)
• Current Registered Nurse (RN) license in the State of Illinois
• Current CPR/BLS Certification
• 3 to 5 years of nursing experience in hepatology, gastroenterology, ambulatory care, care coordination, or a related specialty strongly preferred. Compensation: Salary Range: $39.48
• 65.14 per hour Actual compensation will fall within the range but may vary based on factors such as experience, qualifications, education, location, licensure, certification requirements, and comparisons to colleagues in similar roles.
