Job Description:
- Conduct thorough evaluations of patients to assess mental health needs and substance use patterns to determine appropriate plan of care and treatment needs.
- Develop, implement, and modify personalized treatment plans tailored to the specific needs of each patient. Monitor for changes in status or goal completion.
- Provide counseling, using a variety of therapeutic modalities, to patients with co-occurring substance use and mental health diagnoses in group/individual sessions to assist the patient with successful transition a community-based treatment model.
- Provide psychoeducational to patients and their families about co-occurring disorders, considerations for treatment, strategies for symptom management, and available community-based resources.
- Collaborate with a multidisciplinary team of psychiatrists, psychologists, case managers, and nurses to plan and coordinate treatment to best meet the patient’s needs.
- Maintain accurate and timely documentation of all assessments and interventions in compliance with agency and regulatory standards.
- Engage in regular professional development activities and continuous education opportunities in accordance with relevant licensure requirements.
