ROLE: The Host Homes Case Manager provides a high level of wraparound and social service support to Youth Participants and Host Families in the Host Homes Program. This role works collaboratively to prepare and support families in hosting youth experiencing housing instability, in addition to providing case management to youth participants with goals related to stable housing, education, employment, financial literacy, health, mental health, interpersonal relationships, and general self-sufficiency. Work Schedule

Weekly Hours: 40 FTE: 1.0 (Full-Time) Classification: Non-Exempt Employment Type: Temporary Assignment Duration: The assignment is expected to last approximately five months, with an anticipated duration of fewer than six months from the actual start date. Work Schedule: The typical work schedule is Monday through Friday, with specific hours based on program needs. Occasional evening or weekend work may be required, as well as infrequent out-of-town travel. Approximately one month of training and overlap is anticipated before the Case Manager assumes independent responsibility for the existing caseload. COMPENSATION Expected Hiring Range: Candidates hired for this position can expect a starting wage of $27.30 – $30.00 per hour. The final starting rate will be based on relevant qualifications and experience. Full Position Wage Range: The established wage range for this position is $27.30 – $33.30 per hour . This broader range reflects potential wage progression over time and is not the expected starting range for new hires. Eligible for Vacation, Paid Holidays, and Paid Sick Leave in accordance with organizational policy and applicable law. Work Location

This is a hybrid position based at TOGETHER!'s headquarters and various Host Homes program locations throughout Thurston County. Work locations may include host family residences, schools within the North Thurston, Tumwater, and Olympia School Districts, community partner locations, and an approved remote work location. Regular travel throughout Thurston County is required. Reliable transportation is required for travel between assigned work locations and for transporting youth participants in accordance with organizational policies.

Key Responsibilities

Direct Service & Case Management

• Provide specialized services to youth and young adults in education, employment and career development, housing, and other designated areas, and support them in achieving their goals.
• Manage a caseload of approximately 8–10 youth throughout the lifecycle of the program. Use TOGETHER!'s intake, follow-up, and other data tracking systems to track youth progress and maintain organized records for reporting requirements.
• Provide case management to Traditional and Kinship Hosts throughout the lifecycle of the program.
• Build strong relationships with Host Homes participants, focusing on mentorship and trauma-informed care to effectively provide quality services.
• Develop and maintain individualized, strengths-based, youth-driven service plans.
• Teach life skills, job readiness, and other relevant subjects to youth and families.
• Build and maintain collaborative relationships with funding liaisons, community service providers, businesses, and educational institutions related to specialized services.
• Work with school staff, community members, and partner agencies to support youth success in the program. Develop and nurture relationships with these sectors.
• Transport youth participants in a personal vehicle to Individual Service Plan-related appointments and activities in accordance with TOGETHER! policies and Youth Program Procedures.
• Participate in school district student intervention teams as needed.
• Participate in immigration-related advocacy as needed.
• Recruit and educate interested Hosts through collaboration with program leadership, school districts, and community partners.
• Train and manage Hosts to serve in the Host Homes Program effectively and sustainably.
• Screen and co-interview host families and prospective Host participants. Conduct home inspections, reference checks, and training for Hosts. Administrative Operations Support
• Maintain clear, professional, and timely participant contact notes, service plans, progress updates, outcomes, and other documentation in designated databases.
• Maintain data processes and credentials to access internal and external databases.
• Track, monitor, and report monthly financial expenditures related to case management, direct services, and programming.
• Coordinate and track check requests for Host reimbursements and youth stipends.
• Attend biweekly Program Team Meetings.
• Attend regular, ongoing trainings on topics relevant to the Host Homes Program, including supporting LGBTQ+ youth and youth of color experiencing housing instability, positive youth development, trauma-informed care, and Host Homes model best practices.
• Attend TOGETHER! agency training sessions and staff meetings. General Duties
• Represent TOGETHER! and the Host Homes Program at public events as needed.
• Occasionally provide supervision and training to newly hired case managers, volunteers, student interns, and others to ensure adherence to Host Homes Program policies and procedures.

Required Qualifications

Education Bachelor's degree in Community Health, Social Work, Psychology, Social Sciences, Education, or a closely related field. This requirement may be waived for individuals with four or more years of intensive youth case management experience. Professional Experience

• Minimum of one year of employment experience in case management, social services, health, or education.
• Experience working with children and families facing significant challenges and the ability to work with individuals from diverse cultural and ethnic backgrounds. • Valid Washington State Driver's License and auto-liability insurance.
