Scope of Role

Reporting to the Supervisor, Behavioral Health services, the Specialist, Crisis Intervention & QMHS operates primarily in the community and provides community psychiatric support, rapid response, screening, de-escalation, crisis intervention, resource linkage, transportation, and care coordination services for individuals experiencing behavioral health needs or a behavioral health crisis. The Specialist, Crisis Intervention & QMHS supports the development and execution of treatment plans, helps clients and families access community resources, provides transportation when clinically appropriate, and assists individuals in resolving crises or connecting to assessment and treatment in an appropriate setting. The Specialist, Crisis Intervention & QMHS fosters positive relationships with clients, families, support systems, Signature Health team members, stakeholders, and referral sources while representing Signature Health in a professional and respectful manner.

How You’ll Succeed

  • Develop, prepare, and present treatment program work material and skill-building interventions to clients.
  • Operate primarily in community-based settings, including client homes and other locations in the community, in accordance with treatment plans, safety guidelines, and agency policies.
  • Identify and support client needs related to education, medical care, vocational goals, housing, daily living skills, recovery supports, and community stability.
  • Encourage, monitor, and follow up on client engagement and participation in treatment plan activities.
  • Provide clinically appropriate transportation in a personal vehicle to and from medical, behavioral health, community resource, and essential service appointments.
  • Provide rapid response, de-escalation, screening, and crisis intervention to individuals experiencing behavioral health crises in community, home, and facility settings.
  • Work with individuals in crisis to problem-solve, identify solutions, and arrange assessment or treatment in an appropriate setting when needed.
  • Engage family members and support systems in safety planning and the crisis resolution process.
  • Assist in creating immediate plans to maintain safety and support crisis stabilization.
  • Assist in crisis prevention and early intervention in the community.
  • Coordinate care and link individuals to appropriate treatment services, recovery resources, housing support, medical care, benefits, and other community resources.
  • Provide follow-up contact after crisis episodes according to protocol to check on referrals, assess needs, and support continuity of care.
  • Serve as a liaison with inpatient units and community providers to support treatment engagement and discharge planning.
  • Collaborate effectively with treatment teams, agency staff, departmental staff, stakeholders, referral sources, and community partners.
  • Participate in community outreach and engagement activities.
  • Maintain electronic client charting, administrative files, reports, spreadsheets, and correspondence in accordance with Medicaid, DBH, CARF, and professional standards.
  • Participate in quality assurance, utilization review, staff meetings, clinical meetings, administrative meetings, supervisory meetings, and staff development programs.
  • Meet all productivity, documentation, attendance, punctuality, and workpace expectations.
  • Maintain clear communication and professional boundaries with all clients.
  • Complete trainings as assigned and perform other duties as assigned.
  • Represent Signature Health in a professional manner, including respectful interactions with patients and team members and adherence to appearance expectations. Requirements KNOWLEDGE & EXPERIENCE ______________________________________________________________
  • High School Diploma required. Bachelor degree in a related social services field preferred.
  • Qualified Behavioral Health Specialist (QBHS) designation or Peer Support Specialist (PSS) with at least one year of experience in substance use and mental health with minimum 2 years in recovery and plan for maintaining recovery is required. Crisis experience preferred.
  • Valid unencumbered Ohio driver’s license and proof of driver’s insurance required at all times.
  • Minimum one year case management, crisis, peer support, or relevant behavioral health experience preferred.
  • CDCA preferred.
  • American Heart Association (AHA) Basic Life Support (BLS) certification required.
  • Demonstrated knowledge of the local mental health system, community organizations, community resources, ASAM levels of care, residential services, detox, supported housing, and recovery support.
  • Basic computer skills to perform job duties including desktop computing, email, time sheet management, electronic health records, using Microsoft Office Applications, ADP and other relevant software required.
  • Comfort and experience delivering services in community-based settings and client residences, including client transportation using a personal vehicle.

  • Work may be performed in a variety of settings, including community, home, and facilities.
  • Work involves potential exposure to patients experiencing agitation, which may increase risk for disruptive or aggressive behavior.
  • Flexibility to work evenings and extended hours is required.
  • Requires periods of sitting, standing, telephone, and computer work.
  • Hearing: adequate to hear clients or patients in person, over the telephone or through telehealth technology.
  • Speaking: adequate to speak to clients or patients in person, over the telephone or through telehealth technology.
  • Vision: Visual acuity adequate to perform job duties, including reading information from printed sources and computer screens.
  • Physical effort required: occasional lifting and carrying items weighing up to 15 pounds, unassisted.
  • Possible exposure to blood borne pathogens while performing job duties.
  • Frequent bending, reaching, and repetitive hand movements, standing, walking, squatting and sitting, with some lifting, pushing and pulling exerted regularly throughout a regular workday.
  • Sufficient dexterity to operate a PC and other office equipment.
