Job Summary Under direct supervision of a pharmacist and in accordance with specific and detailed instructions, dispenses and compounds prescribed medications for patient care in keeping with New York State Law. Packages, labels, manufactures, and distributes pharmaceuticals in all areas of the department including inpatient drug distribution, ambulatory services, sterile product preparation, and service to other departments. Reviews and monitors medications and orders written by clinicians to determine safety and efficacy, in addition to answering drug information questions. Other duties include inventory control, patient care area inspections, replenishing emergency boxes, and ensuring the safe and appropriate patient use of drugs, collection of clinical information. Perform other related duties and utilizes automated and computerized technology as required. Essential Functions
- 1. Assist pharmacist(s) with interpretation of medication orders to ensure accurate doses and frequencies. * Demonstrates knowledge of pharmacotherapy, identifies issues appropriately, with an attention to detail.
- 2. Ability to use clinical information systems for retrieval or review of patient information, report generation, and label printing. * Accesses patient profiles with ease and in compliance with HIPAA standards, print reports and labels as well as the disposal of patient health information, and interpretation of reports.
- 3. Communicates with other healthcare personnel. * Information is accurate and communicated appropriately and politely.
- 4. Collects patient specific clinical information, age, weight, height, laboratory data, diagnosis or indications. * Information transcribed accurately.
- 5. Prepares, labels, and delivers patient medication cassettes and unit doses in order to provide drug therapy. Ensures cassette floor stock meet par levels. Census discrepancies rectified through the use of census and transfer reports. * Cassettes and medications prepared without errors and on a timely basis in accordance with center guidelines and procedures and quality improvement standards. Cassette labels are typed and include two patient identifiers. Cassette floor stock is not in over or short supply
- 6. Prepares sterile and non-sterile products and parenteral solutions, including TPN, syringes, and pediatric syringes according to protocol or worksheet for pharmacist verification. * Adheres to aseptic technique, products prepared in laminar airflow hood, compounding records are fully completed and solutions meet medical center quality assurance and improvement standards.
- 7. Ability to use automated dispensing cabinets for medication refill, information review, loading, report printing, refills pockets accurately, uses scanner, repairs pockets and pharmacy console for information review and report generation. * Pockets are refilled with zero error, inventory counts are accurate, broken pockets are repaired, reports are correctly printed, and all expired or deteriorated medications are removed.
- 8. Maintain accurate files, statistics, orders, reports and records pertaining to medication use and requests. * All statistics are properly recorded; order, fill lists, active medication profiles, requisitions and other records are filed in accordance with policy and procedure.
- 9. Prepares and delivers controlled substances, loads or unloads inventory from automated dispensing cabinets, enters requests into business information system, accepts outdated inventory, completes controlled drug records and ensures appropriate signatures are received. * Controlled substance records and business information entries are completed with 100% accuracy, controlled drugs are delivered in a secured manner, all signatures are appropriately received, outdated medication returned to pharmacy and appropriate form completed and no discrepancies created in automated dispensing cabinets.
- 10. Maintains an inventory of drugs and supplies in assigned work area, ensuring proper security and storage, checks routinely for outdated or unusable inventory, returns all unused medications to stock and maintains work area and entire pharmacy in clean and orderly condition. * Pharmacy work areas adequately stocked or not overstocked with no outdated or unusable inventory and kept clean at all times.
- 11. Assists customers at door/windows and answers telephone politely and professionally. * Customer services provided timely and in a courteous manner, phone picked up quickly, states name and location, forwards or escalates call to pharmacists or manager when necessary.
- 13. Records daily workload statistics. * Workload statistics are documented on log sheets with no missing information.
- 14. Delivers medications. * Medications delivered timely and placed in appropriate location on patient care areas.
- 15. Performs activities with the manufacturing, packaging and labeling of medications including unit dose packaging or oral solids, liquid and bulk packages. Utilizes automated packaging system. * 100% compliance with guidelines, policies and procedures, production logs completed, records use of equipment and maintain cleanliness of work area. Operates automated packaging technology.
- 16. Files paperwork including fill lists, packaging records, prescriptions, and medication orders appropriately. * Paperwork placed in appropriate location, no paperwork left out at the end of shift.
- 17.Ambulatory prescriptions filled under the supervision of the pharmacist. Submits claims and verifies payment with PBMS. * Prescriptions entered in ambulatory prescription computer, filled without error, directions for use correct, all required signatures on records, logs off computer after prescriptions are filled. Submission forms are 100% accurate and complete, reviews reimbursement records for correctness.
- 18. Operates carousel technology, pulls medication for new orders, automated dispensing cabinet refills and fill lists, restocks medication received from wholesaler and returns from patient care areas, runs stock out reports, transfers medication from the storeroom, perform cycle counts to ensure accurate inventory levels and expiration date checking. * Uses scanner when removing and adding medications to carousel technology, medications filled into dedicated space without error, reports are run correctly and timely, inventory levels are accurate, removes expired or unusable medication when identified Qualifications: High School diploma/GED. Must be enrolled as a full time student in an accredited school of pharmacy and graduating in 2027 or 2028. Completed first professional year of pharmacy school. Is eligible to receive a NYS pharmacy intern permit. Must be able to work weekends and or holidays. Montefiore Medical Center is an equal employment opportunity employer.
