Employment Type: Full time Shift: Day Shift Description: Discharge Planner / C2 St. Peter's Hospital Full Time Days St. Peter's Hospital is searching for a full-time day shift Discharge Planner to join the compassionate Care Coordination team! The Discharge Planner tasks include, but are not limited to the following:
- Provides discharge planning as a member of an interdisciplinary team.
- Provides discharge planning, referrals, coordination with community providers and provision of other treatment related duties.
- Responsible for patient progression of care, interviewing patients for initial discharge plans and coordinating care to the next appropriate site of care. Education Requirements :
- Bachelor's degree in related field required, or associate degree in related field with QHP required . Experience Requirements :
- Experience in a healthcare/medical setting, preferred.
- Strong writing and communication skills are required.
- Experience working with a diverse population and a strong understanding of multicultural issues is preferred. "Pay Range: $22.25 - $32.30 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
