As a Nurse Patient Liaison/Care Coordinator in our Medical Practice , you’ll be at the heart of that mission. What You’ll Do
- Serve as the primary point of contact for a designated medical provider, managing day-to-day coordination and communication
- Ensure providers are fully prepared for their appointments with organized schedules and up-to-date patient information
- Deliver exceptional client service by educating patients, managing follow-ups, and facilitating post-visit needs like labs, supplements, and referrals
- Communicate with warmth, clarity, and attentiveness to ensure seamless continuity of care
- Coordinate with internal and external providers, retrieve medical records, and help relay wellness recommendations in line with our whole-person approach
- Support team operations by stepping into clinical or administrative roles when qualified and needed Why nurses love working at PALM Health:

- Exposure to advanced therapies and services Requirements Required Qualifications Bachelor’s degree (B.S. or B.A.) or Licensed Practical Nurse (LPN); Social Work background will also be considered One years’ medical practice experience (required); will consider other combinations of education and experience Experience with medical terminology and working in a clinic or hospital setting Strong computer and word processing skills EMR and scheduling experience Why You’ll Love Working at PALM Health Join a mission-driven organization where well-being is a way of life.
