Primary Care Physician

Job Description Summary ‎ Provides comprehensive physician services for patients across the lifespan in ambulatory and community-based care settings. Evaluates, diagnoses, and manages acute illnesses, chronic diseases, and preventive healthcare needs using evidence-based medical practice, including comprehensive health assessments, diagnostic evaluation, medication management, preventive services, chronic disease management, and care coordination. Partners with advanced practice providers and interdisciplinary care teams to improve patient outcomes, promote population health, and deliver high-quality, patient-centered primary care. The Physician, General / Primary Care exercises full clinical decision authority within specialty, independently accountable for patient outcomes within the established care area. Decisions are complex and rely on specialty training and depth of clinical knowledge. This job may include Associate Medical Director responsibilities, influencing clinical practice for a service line, mentoring advanced practitioners, and overseeing chart audits for APPs. ‎ How will you make an impact & Requirements ‎ Key Responsibilities

• Evaluate, diagnose, and manage patients presenting with acute illnesses, chronic diseases, and preventive healthcare needs through comprehensive history, physical examination, diagnostic interpretation, and evidence-based medical decision-making.
• Develop and oversee individualized treatment plans that include medication management, preventive services, immunizations, health screenings, lifestyle modification, chronic disease management, and ongoing monitoring of clinical outcomes.
• Diagnose and manage a broad range of primary care conditions including hypertension, diabetes mellitus, hyperlipidemia, coronary artery disease, asthma, chronic obstructive pulmonary disease, thyroid disorders, obesity, depression, anxiety, musculoskeletal conditions, infectious diseases, and other acute and chronic illnesses using current clinical practice guidelines.
• Order, perform, interpret, and integrate laboratory studies, diagnostic imaging, electrocardiograms, point-of-care testing, and other diagnostic evaluations to establish diagnoses and guide treatment decisions.
• Perform office-based procedures consistent with specialty training and organizational credentialing, including minor skin procedures, wound management, joint and soft tissue injections, incision and drainage, and other approved primary care procedures.
• Conduct annual wellness visits, preventive screenings, health risk assessments, chronic care management, transitional care management, and evidence-based preventive services to promote wellness and reduce disease burden.
• Coordinate care for patients with complex medical conditions, collaborate with specialists and community resources, and determine the need for specialty consultation, hospitalization, or additional diagnostic evaluation when clinically indicated.
• Collaborate with advanced practice providers, nurses, pharmacists, behavioral health providers, care managers, social workers, and other healthcare professionals to coordinate comprehensive patient care across the continuum.
• Educate patients and caregivers regarding diagnoses, medications, preventive health measures, nutrition, exercise, chronic disease self-management, and treatment plans to support informed decision-making and long-term health.
• Document patient encounters, medical decision-making, procedures, treatment plans, quality measures, and clinical outcomes accurately within the electronic health record while maintaining compliance with organizational standards, regulatory requirements, payer documentation guidelines, and value-based care initiatives.
• Participate in quality improvement initiatives, peer review activities, population health programs, patient safety initiatives, clinical pathway development, and continuing medical education to support excellence in primary care delivery. ‎ Compensation Range: $200,801.00 to $301,200.00 The anticipated base salary range represents the Company's good-faith estimate of the compensation it reasonably expects to pay for this position at the time of posting. Actual compensation will be determined based on factors including experience, skills, qualifications, geographic location, internal equity, and business needs.
