Job Description : Job Summary Responsible for collecting and analyzing patient data to formulate individualized care plans. Determines progress toward outcomes and coordinates care delivery. Promotes a safe practice environment and advocates mutual respect with patients, families, and healthcare providers. Essential Functions
- Conducts in-home assessments and develops, implements, and evaluates individualized care plans in partnership with patients, representatives, and caregivers.
- Communicates significant clinical changes or findings to physicians and other healthcare practitioners involved in the plan of care.
- Educates patients and families on disease management, medication administration, and preventative care.
- Monitors vital signs, observes for changes in health condition, and provides wound care as needed.
- Collaborates with healthcare professionals to ensure holistic and coordinated patient care.
- Maintains accurate and timely medical records and documentation.
- Supervises and instructs home health aides and student nurses as directed.
- Takes on-call responsibilities during evenings, nights, holidays, and weekends on a rotating basis.
- Performs other duties as assigned. Education Requirement Associate’s degree in nursing Bachelor’s degree in nursing (BSN) is preferred Experience Requirement 0-2 years of experience in nursing, or a relevant area An equivalent combination of education and experience may be considered. All experience must be paid and in the same related field. Part-time and PRN experience will be prorated based on hours worked per week.
