Summary The primary purpose of the Licensed Practical/License Vocational Nurse (LPN/LVN) GS 6 is to perform assigned duties in a manner that promotes quality health care, positive customer service and the maintenance of an environment committed to serving the veteran. The LPN/LVN also performs assigned duties in a manner that promotes quality health care, outstanding customer service, and the maintenance of an environment committed to serving the Veteran. Duties Duties of the Licensed Practical/Vocational Nurse (LPN/LVN) may include, but are not limited to: Possesses the ability to work with the nursing staff, providers, and others and is able to communicate effectively with patients/family members, and members of the health care team. Is accountable and responsible for data collection, reporting accurate vital signs, height & weight, and performing clinical reminders at time of visit. Adheres to established policies, standards of care and standards of practice. Demonstrates knowledge, skill, and ability to promptly recognize and effectively intervene in potentially urgent or emergent medical and mental health or behavioral situations. Identifies significant changes in the patient's condition and reports to the provider and/or RN Care Manager/charge nurse. Implements nursing interventions based on scientific principles. Performs all Clinical Reminders during patient visit/walk-ins and notifies PCP of positive screens such as ADUIT-C alcohol screen and Tobacco screen. Instructs patients on FOBT procedure for colorectal screening and provides patients with tubes and labels to mail back to facility. Takes vital signs, Height, and Weight with accuracy. For hypertension, records all 3 BP readings in CPRS and notifies provider/Charge Nurse. Assists PACT team with clearing secure messaging within scope of practice and make post-discharge phone calls as indicated. Other duties as assigned/delegate by Charge Nurse/RN. Performance of complex therapeutic procedures requires frequent observation and guidance from the RN Care Coordinator or charge nurse. Reports and records assigned nursing care including but not limited to: Vital signs including Pain as a 51h vital sign; Dressing change, simple and complex; Oxygen administration; Urinary catheterization; Pulmonary hygiene and drainage; Administration of delegated procedures and treatments; Administration of medications; IV Starts; Phlebotomy; Allergy screen; EKGs; Collection of specimens; Nutritional support; Tracheal suctioning; Colostomy care; Clinical reminders with appropriated consults and alerts as indicated. Assists with minor procedures, including obtaining and arranging necessary equipment, positioning patient, ensuring comfort and privacy. Monitors patient during the procedure. Prepares and administers prescribed medications (oral, topical, subcutaneous, intramuscular and/or intravenous) and performs treatments according to established policies/procedures. Recognize urgent and emergent patient care situation, seek assistance of the RN and/or MD/DO, and initiate appropriate emergency interventions as directed. Establish constructive relationships with individual patients and their families to elicit feelings and attitudes, and to promote positive relationships, communication and socialization skills. Records and reports according to MEDVAMC policy, pertinent clinical observation changes in the medical record. Appropriately seeks assistance and guidance by informing the RN Care Manager/charge nurse or medical provider of changes in the patient's condition requiring higher level intervention. Documents clinical reminders and other activities (i.e.: telephone calls) accurately using appropriate templates, diagnosis, and procedure codes. Interacts and communicates effectively with patient/family/significant others, multidisciplinary team members, and immediate staff co-workers. Work Schedule: Monday-Friday, 8:00 a.m.

Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VAInsurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement) Qualifications Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the National League for Nursing Accrediting Commission (NLNAC) at the time the program was completed by the applicant. Exceptions include: Health care education in the military service or training in the military service which is accepted by the licensing body in the jurisdiction in which the individual is licensed as qualifying for full LPN/LVN licensure will be accepted as meeting the education requirements for VHA employment. VHA employees who were converted to appointments under 38 U.S.C. 7401(3) in implementation of Public Law 98-160, enacted November 21, 1983, and for VHA employees converted to appointments under 38 U.S.C. 7405 in implementation of Public Law 99-576, enacted October 28, 1986; and for their future personnel actions under this authority. Full, active, current and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or District of Columbia. An LPN/LVN who has or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions. English Language Proficiency: Licensed practical or vocational nurses appointed to direct patient care positions must be proficient in both spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d). Grade Determinations: In addition to the basic requirements stated in section A, the following qualification criteria must be met for each grade.
