CHIMO: Where What You Do Matters for A Lifetime. About the Program The Solo With Supports (SWS) Program helps youth build the life skills, confidence, and independence they need as they transition into adulthood. Youth may live in a mentor support home or in CHIMO's Transition Home while receiving individualized support. As a Youth Coordinator, you'll support a caseload of youth by providing guidance, case management, and hands-on support to help them reach their goals. What You'll Do:
- Help youth build the skills and confidence they need to thrive as adults.
- Develop, monitor, and update Safety Plans and Service Plans.
- Work closely with youth, families, support networks, and community partners to provide coordinated care.
- Connect youth with community resources and advocate for their needs.
- Keep accurate case notes, reports, and documentation. What You'll Bring:
- A diploma or degree in Child and Youth Care, Disability Services, Social Work, Psychology, Community Rehabilitation, or a related field.
- At least two (2) years of experience working with children, youth, or young adults. Requirements
- Eligible to work in Canada.
- A current Police Information Check and Intervention Record Check (issued within the past six months).
- A valid Alberta Driver's License.
- At least three (3) years of driving experience with a clean three-year Driver's Abstract and Driving Record, with no at-fault claims .
- Access to a reliable vehicle, as regular driving in the community is required. P hysical Requirements
- Ability to lift up to 50 lbs.
- Ability to participate in recreational activities and physically engage with youth when needed. You'll Be a Great Fit If You

- Have experience supporting youth with diverse needs, including developmental disabilities, mental health challenges, trauma, addictions, or barriers to community resources.
- Are organized, adaptable, and comfortable writing reports and maintaining documentation.
