Licensed Vocational Nurse Surgical Specialty Clinics

Summary The LVN is a position within Nursing Services at VA San Diego Healthcare System. The LVN functions as a member of the multidisciplinary team assisting with the delivery of care to patients. The LVN is accountable for the quality of nursing care he or she provides to patients and utilizes the nursing process, assumes responsibility for planning, implementing, directing, and evaluating nursing care for assigned patients. ***Position located in Oceanside VA Clinic*** Duties The LVN assumes the following responsibilities: 1. This position requires flexibility in schedule and assignments, as this is a 24/7 hospital. 2. BLS certified. 3. Obtains/collects specimens as ordered. 4. Participate in performance improvements. 5. Administers medications and treatments to patients with complex nursing problems. 6. Participates in daily DMS huddles and unit specific staff meetings. 7. Participates in coaching and mentoring of colleagues or new employees. 8. Performs and documents within the electronic medical record. 9. Completes CPRS documentation and closes encounters by facility policy. 10. Provides timely patient follow-up. 11. Maintains confidentiality of patient information. 12. Seeking opportunities for self-improvement and participating in performance improvements. 13. Collaborates and communicates effectively with the Interdisciplinary Care Team. 14. Documentation is factual, objective, accurate, consistent, timely, and complete. 15. Documents and signs all patient related activity within tour of duty. 16. Implements nursing interventions/orders in a timely and complete manner. 22.Recognizes changes in patient's condition and deviations from baseline that require immediate intervention. 23.Provide patient education related to managing risk factors, medications, procedures, and other medical regimens related to disease process/specialty clinic, within scope of practice. 24.Educates patients on provided discharge instructions related to their disease process, ensuring the patient can ask questions. 25.Recognizes changes in patient's condition and deviations from baseline that require immediate intervention. 26. Apply infection control measures appropriately in delivering care. 27.Alerts providers (physicians/nurse practitioners) of abnormal signs and symptoms and/or abnormal test results to expedite care. 28.Collaborate with the Registered Nurse or Charge Nurse when the patient requires a higher level of care that is not manageable on the unit. 29. Obtains Nail Certification within 12 months of hire. 30. Performs other duties, as assigned. Surgical Specialty Clinics: 1. Provides direct patient care to Surgical Specialty patients in accordance with hospital policies and practice standards and guidelines. 2. Gathers additional data (face to face or via phone) related to patient needs for treatment determination of Surgical Specialty Clinic's needs. Participates in ongoing quality improvement workplace activities, including conducting unit audits, as appropriate to the individual's position, education, and practice. 3. Participates in Surgical Specialty performance measure activities such as, but not limited to, hand hygiene, universal protocol, and access to clinics. 4. Assists RN with coordinating care of patients as needed, including patients consulted. 5. Assured patients have pre-appointment requirements completed prior to Surgical Specialty procedure. Preferred Experience: 1-2 Current experience in Ambulatory Care (PACT, Surgical Specialty). Experience in gynecology, podiatry, orthopedic, ED, inpatient care helpful. Licensure

• Active, full, current and unrestricted licensure as a Graduate Licensed Practical or Vocational Nurse in a State, Territory, or Commonwealth (i.e., Puerto Rico), of the United States or the District of Columbia. Grade Determinations: In addition to the basic requirements stated above, the following qualification criteria must be met for each grade. The candidate's qualifications must clearly demonstrate the level of competence required for the grade. GS-03 None beyond the basic requirements. LPNs at this grade level are expected to perform all duties within the scope of their license (e.g., medication administration); however, completed work may require routine or detailed higher level review depending upon the complexity of the duties involved. GS-04 (a) Six months of qualifying experience as an LPN or LVN; or(b) Graduation from an approved school (which may be waived as provided in paragraph 4 of Section C) and one year of experience that involved nursing care work in a hospital, outpatient clinic, nursing home, or other supervised medical, nursing, or patient care facility that provided a practical knowledge of human body structure and sterile techniques and procedures; or (c) Graduation from an approved school of at least 24 months duration. GS-05 Completion of at least 1 year of qualifying experience at the GS-4 level or equivalent Knowledge and ability to provide a full range of practical nursing care to patients with a variety of physical and/or behavioral problems. Ability to serve as a responsible member of a nursing team and interact in an appropriate manner with patients, family members, professional and/or other support personnel involved in the delivery of patient care. Knowledge and skill sufficient to prepare, administer, and appropriately document actions taken specific to commonly prescribed oral, topical, subcutaneous, intramuscular and/or intravenous medications. This includes observing and documenting patient responses to medication and reporting noted changes in the patient's condition. Knowledge and ability to recognize the need for and to institute emergency measures and assist in resuscitation procedures in cardia and/or pulmonary arrest. Recognize and appropriately respond to breakage/malfunction or loss of equipment, safety hazards, and supply deficiencies.
