Summary This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Duties The Community Living Center (CLC) Registered Nurse (RN) is responsible for providing competent, evidence-based care to residents with oversight of licensed vocational nurses/nursing assistants as appropriate to the setting. The nurse will assist in directing the provision of nursing education, orientation, competencies and providing quality improvement and enhance outcomes utilization. Executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex resident care. Collaborates in the development, implementation, evaluation, and revision of policies, procedures, and/or guidelines. Demonstrates performance and leadership that is broad enough to improve the care for a group of residents. Provides peers with formal or informal constructive feedback for improvement. Supports colleagues and other nurses through knowledge sharing to provide safe, quality nursing care. Fosters a safe and supportive environment conducive to the professional development of health care professionals. collaborates with the Minimum Data Set (MDS) coordinator to ensure accuracy of the Resource Utilization Group (RUG) score. Maintaining consistent assignment, ensuring continuity of care to as much as possible. Completes and accuracy documents physical assessments on admission, to include/but not limited to: all body systems and assigned assessments designated in the CLC. Accurately assesses changes in the Resident's abilities (age related, other injuries, cognition etc.)and medical situations (new colostomies, enteral feedings, prosthesis etc.) and acts appropriately to teach and implement new skills to promote Resident independence, safety and social interactions. Communicates and interacts appropriately and courteously with all internal and external customers. Maintains confidentiality of electronic, written, and/or verbal Resident/employee information. Assures that Restorative Care measures are implemented by Team Members. Actively collaborates with Interdisciplinary Team (IDT) in establishing, maintaining and updating an individual written plan of care for each Community Living Center Resident. Provides skilled Nursing care to Community Living Center Residents and families within the scope of accepted nursing standards of practice. Performs a comprehensive initial assessment of the Veteran's disease process and/or co-morbidity factors, including medical history, medication and allergy history, Veteran's symptoms, pain rating and psychosocial assessment. Completes all documentation in accordance with regulations and Agency policy and procedure. Performs ongoing assessments of Veteran's disease process and updates the plan of care as necessary in consultation with the physician and other members of the IDT. Perform ongoing assessment of the impact of the terminal diagnoses on the Veterans physical, functional, psychosocial and environmental needs and activities of daily living. Coordinates care with other healthcare providers, including spiritual counselor. Medical social worker, bereavement services and rehab services as appropriate. Educates the Veteran/family regarding the disease process, self-care techniques, end of life issues and the process for resolution of care and complaint issues. Collaborates with staff member's regarding Veterans' needs a necessary. Assist in the development of plans of correction and integrate recommendations into staff's daily function. Actively participates in Long-Term Care Institute (LTCI) readiness and CLC Compare performance improvement activities Other Duties as Assigned. Preferred Experience: One year of long term care, hospice or rehab experience. Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE); OR Individuals attending a master's level bridge program in nursing who have completed coursework equivalent to a bachelor's level degree in Nursing may have opportunity to become registered as a nurse with a state licensing board prior to completion of the bridge program. Upon achievement of a State license, the individual may be appointed on temporary basis and later converted to a permanent appointment upon successful completion and graduation from the bridge program. (Reference VA Handbook 5005, Appendix G6); OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active, and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing to warrant an appointment as a Nurse who has completed an associated degree/entry level Nursing education program. Credit for foreign nursing education higher that associate degree/entry level requires a formal degree equivalency validation from a recognized equivalency evaluation accepted by VA such as International Consultants of Delaware (ICD). Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Graduate Nurse Technician (GNT) Exception: Candidates who otherwise meet the basic education requirements, but do not possess the required licensure, may be appointed at the entry step of the grade and level applicable to the completed nursing education as a GNT on a 120-day temporary appointment while actively pursuing licensure (may be extended up to two years on a case-by-case-basis.)
