Description Sangre de Cristo Community Care is seeking a compassionate and dependable Certified Nursing Assistant (CNA) to join our hospice home care team in Pueblo, Colorado. As a Hospice CNA, you'll have the opportunity to provide meaningful, one-on-one care to patients in the comfort of their own homes. You'll be an important part of an interdisciplinary hospice team focused on helping patients live with comfort, dignity and quality of life. This position works Friday, Saturday and Sunday during the day. What You'll Do Working under the direction of an RN or Case Manager and following each patient's individualized Plan of Care, you will:
- Provide personal care including bathing, grooming, dressing and toileting
- Assist with mobility, transfers, positioning and ambulation
- Provide assistance with meals, hydration and nutritional needs
- Observe patients and promptly report changes in condition to the nursing team
- Help maintain a safe, clean and comfortable environment for patients
- Travel to patients' homes and other care settings throughout the assigned service area
- Provide compassionate support while respecting each patient's dignity, privacy and personal preferences
- Communicate and collaborate with nurses and other members of the interdisciplinary hospice team

- Current Colorado CNA certification in good standing
- At least one year of CNA experience preferred
- Hospice, home health or other direct patient care experience preferred
- Valid driver's license and reliable transportation
- Compassionate, dependable and comfortable providing end-of-life care
- Strong communication and time-management skills
- Ability to work independently while remaining connected to the clinical team
- Availability to work the Friday–Sunday day schedule, including holidays that fall within the scheduled rotation
