Home Care Nurse (Keene Area)

Home Healthcare, Hospice & Community Services is looking for a compassionate and skilled Home Care Nurse (Keene Area) to provide individualized nursing care to patients throughout our community. As a Home Health RN, you’ll use your clinical judgment and nursing expertise to assess patient needs, develop and carry out individualized plans of care, provide education, and coordinate services with an interdisciplinary team. You’ll work collaboratively with physicians, case managers, therapists, aides, and other members of the care team while developing meaningful relationships with patients and families. What You’ll Do
• Perform comprehensive admission and ongoing assessments of patients, including physical, psychosocial, and environmental needs.
• Develop, implement, evaluate, and update individualized plans of care.
• Provide skilled nursing interventions within your professional scope of practice.
• Monitor patient conditions and promptly communicate significant changes to the appropriate members of the care team.
• Educate patients and caregivers about treatment, recovery, symptom and complication management, disease progression, and available resources.
• Help patients and caregivers build the knowledge and skills needed to promote independence and autonomy.
• Coordinate care with case managers, physicians, therapists, aides, and other health care professionals.
• Advocate for access to needed services, equipment, medications, and community resources.
• Maintain accurate, timely, and complete clinical documentation using electronic medical record and mobile technology.
• Participate in interdisciplinary meetings, case conferences, utilization review, and quality improvement activities.
• Delegate appropriate nursing tasks and provide supervision in accordance with professional judgment and regulatory requirements.
• Maintain established productivity standards, including an average of 5–7 patient visits per day .
• Follow agency requirements related to patient safety, infection control, confidentiality, documentation, weekend and holiday care, and regulatory compliance.
• Precept students and new employees as requested and participate in ongoing professional development. What You’ll Need
• Graduate of a state-licensed basic nursing program.
• Current New Hampshire Registered Nurse license in good standing.
• Current CPR certification .
• One year of professional nursing experience under appropriate nursing supervision, or an equivalent combination of education and experience demonstrating the competencies required for the position.
• Valid driver’s license and reliable access to an automobile during working hours that meets New Hampshire registration and automobile liability insurance requirements.
• Ability and willingness to use electronic medical record and mobile field technology while appropriately protecting patient information.
• Strong interpersonal skills, flexibility, and sensitivity when working with patients and families experiencing illness, stress, and end-of-life needs.
• Ability to work respectfully and effectively with individuals and families from diverse cultural, spiritual, religious, and personal backgrounds. Preferred Qualification
• Preparation in community staff nursing through an approved program. What to Expect in the Role This is a community-based position requiring travel to patient homes and other care settings. Home environments vary and may include exposure to animals, smoke, odors, blood and body fluids, medications, needles and sharps, and changing weather conditions. Appropriate personal protective equipment and infection prevention practices are required. The role involves regular movement and hands-on patient care, including standing, walking, bending, kneeling, reaching, pushing, pulling, and assisting with patient care and equipment. The position may require lifting and carrying up to 50 pounds ; greater weights require additional assistance or caregivers as appropriate.
